• Biceps Tendonitis

    Biceps tendonitis may be defined as inflammation or irritation of the long head of biceps tendon. Pain develops at the front of the shoulder and is worsened by lifting weights in front of the body or overhead. It may also be caused by an injury such as a traumatic fall, incorrect weight training, over-training or rubbing of a bone spur on the biceps tendon, with instability of the tendon itself.

    Bicipital tendonitis often occurs in tandem with a rotator cuff tear. Other problems that often accompany biceps tendonitis include:

    •Arthritis of the shoulder joint
    •Tears in the glenoid labrum
    •Chronic shoulder instability
    •Shoulder impingement
    •Other diseases that cause inflammation of the shoulder joint lining

    In the early stages of biceps tendonitis, the tendon becomes red and swollen. As tendonitis develops, the tendon sheath, (covering) often thickens or grows larger. The tendon in these late stages is often dark red in color due to the inflammation. Occasionally, a deformity of the arm occurs, i.e. a “Popeye” bulge in the upper arm.

    The biceps muscle is in the front of the upper arm. It helps bend, (flex) the elbow/shoulder and rotate, (supinate) the elbow. It also helps keep the shoulder stable. The biceps muscle has two tendons that attach it to bones in the shoulder. The long head attaches to the top of the shoulder socket, (glenoid). The glenoid is lined with soft cartilage called the labrum. This tissue helps the head of the upper arm fit into the shoulder socket. The short head of the biceps tendon attaches to the coracoid process, (just below the clavicle).

    Signs and Symptoms:

    *Pain or tenderness in the front of the shoulder, which worsens with overhead lifting or activity.

    *Referring pain that moves down the upper arm.

    *An occasional snapping sound or sensation in the shoulder.  

    *A “Popeye Bulge” on the upper arm.           

    Investigation:

    The Sports Injuries Specialist – Registered Osteopath may recommend an ultrasound scan, which is useful to help diagnose biceps tendonitis and rotator cuff tears. An MRI scan may also be useful if and when deemed necessary.

    In some cases, surgery is necessary to repair the tendon. However this is only deemed necessary when the tear is large enough to warrant such drastic action. In the majority of cases, treatment from the Sports Injuries Specialist – Registered Osteopath is sufficient, allowing the patient to return to full activity as soon as a possible. Homework, including exercises and hydrotherapy will also be given.
